How to obtain Can be obtained by bombing the item pedestal in the first floor's Treasure Room as Tainted Isaac.

Becomes Tainted Isaac's pocket active item when picked up.

Can only be used as Tainted Isaac. Attempting to pick it up as any other character will use Plan C and kill them.

This is an Epiphany unlock challenge item - picking it up will disable achievements for the rest of the run, and display the Epiphany door icon on the HUD.

While heldReplaces almost every item with Blight. Does not affect quest items.

Going into Depths II will force Tainted Isaac to use the Blighted Photo in order to open the Strange Door. This will consume the item.

The Mausoleum/Gehenna floor will contain Child's Drawing instead of Dad's Note - picking it up will teleport Tainted Isaac to an extended version of the Home floor.

The Home floor will contain a Big Chest in the starting room that, when opened, will teleport to a closet room with Tarnished Isaac inside it - touching him will unlock the character and end the run.

The Chest will require a Golden Key to open that can be found in the floor's bathroom.

On useCan only be used when standing next to a Blight item pedestal. The pedestal will be converted into 2 items from the current room's item pool - only one of them can be taken.

Also deals a full heart of damage when a pedestal is converted.
